Insurance and Payment Information

At Cosmos Mental Health Counseling LLP, we are committed to making mental health care accessible and affordable. We understand that navigating insurance and payment options can be complex, so we aim to provide clear information to help you understand your coverage and payment options.

In-Network and Out-of-Network Insurance

We work with a variety of insurance providers to offer you the best possible coverage for therapy services.

Our accepted in-network insurance plans include:

  • Cigna
  • Aetna
  • Healthfirst

Our accepted out-of-network insurance plans include:

  • NYSHIP
  • GHI

If your insurance provider is not listed here, please contact us. We are happy to assist you in determining your coverage options and may be able to provide super bills for insurance reimbursement.

Verification of Benefits

Before your first appointment, we recommend verifying your insurance benefits to understand your coverage and any out-of-pocket costs. To assist with this process, you can:

Contact Your Insurance Provider: Call the customer service number on your insurance card and ask about your mental health benefits, including coverage for therapy sessions, co-pays, deductibles, and any limitations.

Provide Us with Your Information: You can give us your insurance details, and we can help verify your benefits on your behalf. Please contact our office at (631) 296-0887 or scheduling@cosmoscounselingny.com with your insurance information.

Payment Options

In addition to insurance, we offer several payment options to accommodate your needs:

Self-Pay: If you prefer to pay out-of-pocket or if you do not have insurance coverage, we accept payment via credit/debit card. Our self-pay/ private pay rates are $125 for individual sessions and $175 for couples/ family sessions.

Sliding Scale Fees: For those who may have financial constraints, we offer a sliding scale fee structure based on income and financial need. Please contact us to discuss this option.

Cancellation/ No-show

We require a 24-hour notice for canceling or rescheduling appointments. Cancellations with less than 24 hours’ notice or no shows will incur a fee
